Phase I: Static Forms. PDF versions of the forms are available via the Internet on a Web site. Individuals may then print the forms, complete them manually and fax or mail them in. Online help is available to assist individuals.

Phase II: Non-Integrated Dynamic Forms. Individuals access forms on line, complete them on the computer and then submit them electronically via e-mail. Phase III: Dynamic Forms with Single Entity Back-Office Integration. An integrated system that captures and processes information and integrates it dynamically with back-office systems, eliminating human intervention at all stages of the process.

Phase IV: Dynamic Forms with Multiple Entity Back-Office Integration. A constituent-centered approach, providing integration with multiple agencies to streamline complex activities such as claims processing.
